Make an Appointment
The first step in making an appointment is to fill out an Intake Form. This will provide me with your personal and contact information. It will also help me understand your optimal learning channels and any special needs you may have.
Please go to my Contact page and send me an e-mail with a short description of the kind of treatment you seek. I will email the Intake Form to you as an electronic attachment (MS Word File). Please fill it out completely and email it back me, as an attachment. I will review it and call you to discuss your needs and set-up an appointment. At your first appointment, I will have you sign the form. This will give me permission to hypnotize you.
I normally recommend no more than 3 appointments. You must agree to complete the recommended course, otherwise your success will be less than optimum. Smoking takes one appointment, whereas 3 visits are recommended for weight loss. For long term weight loss success, an appropriate diet and/or exercise plan should be in place.
Should you be under a doctor's care for your condition, I will require his or her permission to treat you. This is for your safety. Please ask your doctor to provide permission in writing on a prescription pad, or on his or her stationery.
The cost of an initial individual hypnosis appointment is $100, and $80 for subsequent appointments treating the same condition. The group rate can be extended to groups of two or more people. This cost is $50 per person, per session. Corporate, student and senior rates are also available.
Reiki sessions are available for either one or one half hour. The cost is $65 and $35 respectively.
I respectfully request 24 hour notice should you need to cancel or postpone your appointment for any reason. You will be charged a late cancellation fee at the rate of 50% should you cancel or postpone without giving adequate notice.
